What do the cosmic disturbances in Revelation 6:13 indicate?

They signal supernatural divine judgment during the first half of the seven-year period, when stars fall, heaven departs like a scroll, and mountains and islands move, producing tremendous earthquake activity. This shaking terrifies rulers and people, who hide and plead for death, while the second half will be far worse.

Les explains that when God deals with Israel during these seven years, the supernatural is present almost daily. In verse 13, the cosmic disturbances include the stars of heaven falling to earth, heaven departing as a scroll rolled together, and every mountain and island being moved out of place, which he describes as tremendous earthquake activity. This is only the first half; the second half will be far worse. Even rulers, kings, great men, rich men, chief captains, mighty men, bondmen, and free men are shaken, hiding in dens and rocks and pleading for death. They cry to be hidden from the face of Him on the throne and from the wrath of the Lamb. Les notes that for over 1900 years mankind has not seen the wrath of God; past calamities, earthquakes, and wars were not God's wrath.
